All new patients at Campbell Family Medicine begin with our 6-month Comprehensive Membership Program, designed to give you the full attention and in-depth care you deserve as we build your health foundation. During this NEW PATIENT membership we are working together to find the root cause of your health issues.

• Physical Exam

• Direct email & after-hours phone access to Dr. Campbell and Office Manager

• Initial lab workup (Quest-CHL Laboratory)

• ONE genetic profile test (IntelXXDNA)

• Health coaching (up to 6 group classes + 6 personal visits)

• Body Composition Analysis (BIA) & EKG

• 1 Breast Thermography Screening

• Rapid strep, flu & pregnancy tests

• Vision screening, ear irrigation, urinalysis

• Access to our Loom Library for educational resources

• Preferred pricing on additional labs

Cost & Payment Options

• Initial down payment: $2,599

• Balance: Pay over 5 months or make a lump sum payment

• Total: $9,599

What Happens Next?

After completing your initial 6 months, you’ll automatically transition into our Standard Membership Program at a significantly lower monthly rate (currently $364/month), unless your care needs change.
